About this resource
Young children describe their moms in ways no grown-up would think of, and those answers deserve saving. This Mother’s Day questionnaire and card activity collects them in kindergarten through Grade 2 (Grades K–2), with simple prompts to draw and write and a matching foldable card to send home. It builds Social Awareness through gratitude and naming feelings.

- Questionnaire, “All About My Wonderful Mom” (Grades 1–2): nine longer prompts about what she says, how she helps and why she’s amazing (4 pages)
- Each questionnaire in a fill-in-the-blank format and a visual box layout, in color and black and white, with a “This is me and my mom” drawing space
- A foldable card for each level that prints on one double-sided sheet and folds into a four-page booklet (8 pages)

How to use this Mother’s Day questionnaire and card activity
- Complete the questionnaire first, then the card, and send the SEL worksheets and card home together
- Choose the visual box layout or the fill-in-the-blank format to match your students’ writing
- Use the shorter level with kindergartners and the nine-prompt level with Grades 1–2
- The prompts work for any important caregiver, not only a mom
Grade fit
Short fill-ins and a big drawing space suit kindergartners, and the longer level stretches first and second graders, so the set covers Grades K–2. These printable worksheets keep writing to a few words per line.
