About this resource
Most students can name their mom’s favorite treat without thinking, yet few have ever been asked. This Mother’s Day survey and reflection activity gives Grades 2–4 two pages to write it all down and turn it into a gift. Five caregiver versions mean students can take part without explaining their family, and the activity builds Social Awareness through perspective-taking and gratitude.
What students practice
- Seeing a caregiver through her eyes
- Expressing thanks in a short letter
- Choosing three words that describe someone
- Noticing something new about a person they know well
What’s inside
- 5 caregiver versions: Mom, Grandma, Nana, Auntie and Stepmom
- “All About My Mom”: checkboxes for favorite food, color, treat and weekend activity, a superpowers list, three describing words and sentence starters
- “Thinking About My Mom”: what to thank her for, what students realized while filling out the survey and how they feel when they think of her, ending with a mini letter and a picture box
- Every page in color and in black and white (20 student pages)
- SEL notes and printing tips; 23 pages
A look inside
- “My Mom’s Superpowers”
- “Something I realized while filling out this survey is:”
- “When I think about my Mom, I remember…”
How to use this Mother’s Day survey and reflection activity
- Gratitude worksheets that students turn into a gift for a caregiver
- Print back to back, survey on one side and reflection on the other
- Hand out the Grandma, Nana, Auntie or Stepmom version to any student who needs it
- Checkboxes keep reluctant writers going, and the letter gives stronger writers room to say more
Grade fit
Ticking boxes and filling short sentence starters is within reach for Grade 2, and the reflection page asks Grade 4 students for real thinking. That makes these printable worksheets right for Grades 2–4.
